These are some things we typically need to find out before we take in a device for service. Please take the time to review them carefully, as it will help during check-in.
- If your computer is operational and you have no liquid damage, install our MEADIAcare Diagnostics software to speed up your repair. The diagnostic results will be shared when you come in or after you submit this form. Look for the MEADIAcare icon in your system menu bar or your System Preferences to see if it's installed.
We need the Serial Number of Apple Product to check warranty information and parts.

If you cannot find your serial number, just type "Don't know" or "Unavailable".
Some repairs or service require your Apple ID to run updates or perform repairs.
- You will have an Apple ID if you have ever downloaded an App, music from the iTunes store, or used any iCloud features. If you need further assistance, we can help you at check-in.
Prior to servicing any iOS devoice, FMIP will need to be turned off.
Turn off Find My Device
- Go to Settings > iCloud
- Tap to turn off Find My [device].
If your device no longer powers on, you can disable Find My Device by following the instructions here.
Important: Do not use Find My Device to erase a device you may wish to recover data from. We will not be able to recover data from an erased device.
We may need to contact the other service provider for information during diagnostics.
Warranty coverage changes if your device has liquid damage. If your unit has liquid damage, DO NOT power it on and bring it in immediately after completing the rest of this form. Let us know how long ago it occurred in the description.
Back-up prior to any service is recommended. You can backup your Mac using Time Machine. You can back-up your iOS device via iCloud or iTunes. Click no for instructions on how to preform a backup.
Many repairs require restoration of data after, and there is always a risk of data loss when working with any kind of computer equipment. Backup services are available, and data recovery services are available for devices that no longer power on.
iCloud backups can be done with a wireless Internet connection and an Apple ID, even without access to a computer. Please ensure the iCloud backup completes before bringing in the device. -
How to Configure iCloud backups
iTunes backups can be done in iTunes on a Mac or PC, even if you do not have an iCloud account or Internet connection.
Important warning about encrypted backups: If your password protect your backup, please write down and do not loose the password you set. Recovery of this password is impossible, and all future backups made with your phone will be encrypted with it. Recovering encrypted backups requires the password, as does turning off backup encryption.

to 'yukon' before you bring in your computer. If you are unable to change it, type your password in the "other" choice.

Terms and Conditions
A preliminary diagnostic, checklist and signed work-order must be completed before service. This can take 15-20 minutes. Apple warranty does not apply to issues caused by non-Apple hardware; result of software; no trouble found; data recovery requested; physical damage; exposure to liquid. Data backup prior to work is the sole responsibility of the customer. Apple reserves the right to determine whether your device is eligible for out-of-warranty service. Non-authorised repairs/parts or
liquid damage may be billed for full replacement cost. Non-warranty repairs or replacements have a 90-day limited hardware warranty.
Back-up of your data is the sole responsibility of the customer. We can assist you with back-up solutions or provide data recovery services upon request.
